This Notice of Privacy Practices describes how medical information about you may be used and disclosed, and how you can access this information. Please review this Notice carefully. This Notice applies to all medical information created or maintained by Glendale’s Urgent Care.
Our Commitment to Your Privacy
Glendale’s Urgent Care is committed to protecting the privacy and security of your health information. We are required by law to maintain the confidentiality of your Protected Health Information (“PHI”), to provide you with this Notice describing our legal duties and privacy practices, and to follow the terms of the Notice currently in effect. We reserve the right to change this Notice at any time. Any changes will apply to all PHI we maintain, including information created or received before the change. The most current version of this Notice will be available on our website and at our clinic.
How We May Use and Disclose Your Health Information
We may use and disclose your PHI without your authorization for the following purposes, as permitted or required by law.
Treatment
We may use and share your PHI to provide, coordinate, or manage your medical care. This includes sharing information with physicians, nurses, laboratories, pharmacies, imaging centers, specialists, hospitals, and other healthcare providers involved in your treatment. Information may also be shared with individuals involved in your care, such as family members, when appropriate. If you participate in telehealth services, your information may be shared electronically through secure systems to facilitate your care.
Payment
We may use and disclose your PHI to bill for services provided and to collect payment. This may include sharing information with health plans, insurance companies, workers’ compensation carriers, or other third parties responsible for payment. We may also disclose information to billing services or collection agencies as permitted by law.
Healthcare Operations
We may use and disclose your PHI for healthcare operations, which include activities necessary to run our clinic and improve the quality of care. These activities may include quality assessment, staff training, licensing, accreditation, audits, legal services, and administrative functions. We may also use de-identified information for certain operational purposes.
Appointment Reminders and Communications
We may use your PHI to contact you regarding appointments, test results, or follow-up care. Messages may be left with minimal information when necessary.
Treatment Alternatives and Health-Related Services
We may use your PHI to inform you about treatment options or health-related services that may be relevant to your care.
Uses and Disclosures Required or Permitted by Law
We may disclose your PHI in the following circumstances:
- Public Health Activities: Reporting diseases, injuries, adverse drug reactions, or other public health concerns as required by law.
- Health Oversight Activities: Audits, investigations, inspections, or licensure activities conducted by government agencies.
- Legal Proceedings: In response to court orders, subpoenas, or other lawful processes.
- Law Enforcement: When required for law enforcement purposes or to report crimes.
- Workers’ Compensation: As required to comply with workers’ compensation laws.
- Serious Threats to Health or Safety: To prevent or lessen a serious threat to health or safety.

Right to Access and Copies
You may request to inspect or obtain a copy of your medical and billing records. Requests must be made in writing. We may charge a reasonable fee as permitted by law.
Right to Request Restrictions
You may request restrictions on how we use or disclose your PHI. We are not required to agree to all requests, but we will comply when required by law.
Right to Confidential Communications
You may request that we communicate with you in a specific way or at a specific location. We will accommodate reasonable requests.
Right to an Accounting of Disclosures
You may request a list of certain disclosures of your PHI made by us, excluding disclosures for treatment, payment, or healthcare operations.
Right to a Paper Copy of This Notice
You may request a paper copy of this Notice at any time, even if you have agreed to receive it electronically.
Right to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
Electronic Records and Patient Portals
We may offer electronic access to medical records through patient portals or secure communication systems. While we take reasonable steps to protect electronic information, patients are responsible for maintaining the security of their login credentials.
Business Associates
We may disclose PHI to third parties who perform services on our behalf, such as billing services or IT providers. These parties are required by law to protect your information.
Authorization for Other Uses
Any use or disclosure of your PHI not described in this Notice will be made only with your written authorization. You may revoke your authorization at any time in writing.
No Waiver of Rights
We will not require you to waive your rights under HIPAA as a condition of receiving treatment.
